In partnership with EPIC Entertainment & Sports, USA Field Hockey offers a variety of insurance programs for athletes, clubs, coaches and officials.   • Report an Incident

    Club Officials, Coaches, or Event Directors should complete the Incident Report form at the time of the occurrence during a covered activity, as this ensures accuracy in reporting and may help with any liability claims that could arise from the incident. Once the form is completed, the applicable individual should fax or email the report to USA Field Hockey. The form provides more information on how to submit.

    If necessary, USA Field Hockey will provide an Accident Medical Claim form to the injured participant with instructions for submission to USA Field Hockey's Participant Accident Insurance provider.

Types of Insurance

  • General Liability Insurance

    The USA Field Hockey Commercial General Liability policy provides coverage to club memberships at no additional cost. This coverage protects against liability claims brought by third parties alleging bodily injury, property damage, personal or advertising injury arising from a covered club’s premises, operations, and products during Covered Activities.

  • Participant Accident Insurance

    USA Field Hockey Participant Accident Insurance provides Excess Accident Medical and Accidental Death & Dismemberment (AD&D) benefits to members who are injured while participating in USA Field Hockey Sanctioned Events.  

    Coverage is only applicable for injury during USA Field Hockey sanctioned events, and does not extend to pre-existing conditions or injuries in unsanctioned events. If an accidental bodily injury results in an Insured Person requiring medical care and treatment within 90 days of the accident, the policy pays the reasonable medical charges of necessary medical services up to the benefit amount. 

    Medical expenses must be incurred within two (2) years of the date of accident. The accident medical coverage is secondary to any other available medical/health insurance and is subject to a $500.00 deductible per claim.

  • Directors and Officers Liability Insurance (Not for Profit)

    Directors and Officers (D&O) Liability Insurance is available for purchase on to all Member Clubs through USA FIeld Hockey. D&O Insurance protects club directors and officers from personal liability associated with wrongful acts, errors, and omissions in the performance of duties for the club, including mismanagement of funds, and negligent decisions affecting club members. The policy provides Employment Practices Liability (EPL) coverage for claims alleging harassment, discrimination or wrongful termination. 

    All members, including athletes and coaches, of the Member Club must be members of USA Field Hockey to receive insurance.

  • General Liability Insurance

    Covered Parties:

    • General Liability insurance covers USA Field Hockey and all currently registered members. This includes national team athletes, Nexus regions, directors, officers, Member Clubs, umpires, coaches, contractors, event owners, event organizers and volunteers, while acting in their capacity as such with respect to events or other activities that are sanctioned or approved by USA Field Hockey. USA Field Hockey insurance only covers current USA Field Hockey members. 
    • Please note that to receive insurance coverage all members of Member Clubs must also be members of USA Field Hockey.

    Covered Activities:

    Sanctioned Events and approved activities include:  

    1. Field hockey games, tournaments, practices, camps, clinics, and related or incidental activities that are sanctioned or owned by USA Field Hockey.  
    2. For USA Field Hockey Member Umpires, coverage extends to officiating at any amateur field hockey competition at all levels, including NCAA, high school and others, regardless of if the event is sanctioned by USA Field Hockey. Coverage also extends to serving as an assignor for any field hockey related activities. For more information about Umpire-specific insurance, please click here. 
    3. For USA Field Hockey Member Clubs, coverage extends to club sponsored or supervised activities such as club or team meetings, banquets and usual, non-hazardous fundraisers such as bake sales, car washes, and other similar events. Please note that all members of the club must be USA Field Hockey members to receive insurance. 
  • Participant Accident Insurance

    Covered Individuals:

    • All current USA Field Hockey members are eligible for coverage under USA Field Hockey’s Participant Accident Insurance. USA Field Hockey members are considered current when they have completed all necessary components – waiver, background check and SafeSport training when applicable – of the membership type.

    Covered Activities:

    The USA Field Hockey Participant Accident Insurance policy provides coverage for USA Field Hockey members for accidental bodily, accidental death, and dismemberment directly resulting from participating in USA Field Hockey sanctioned events. Covered activities include: 

    1. Field hockey games, tournaments, practices, camps, clinics, and related or incidental activities that are sanctioned or owned by USA Field Hockey. 
    2. Group travel as a team or club directly to or from scheduled practices, games, sanctioned or approved activities.
    3. Other supervised activities, such as club or team meetings, banquets, and usual, non-hazardous fundraisers. 
  • Directors and Officers Liability Insurance (Not for Profit)

    Covered Individuals:

    • As this policy is optional, only the Member Clubs that enroll in this program are covered. 

    Cost:

    • The cost is $550.00 for a $1,000,000 limit, and $850 for a $2,000,000 limit. The program rates include a $25.00 fee for program administration and payment processing.

Certificates of Insurance 


Certificates are returned within five (5) business days, unless otherwise noted. Certificate Request forms should be submitted to: USA Field Hockey's Membership Department, either by email at membership@usafieldhockey.com or through the USA Field Hockey Membership System by clicking Request a Certificate on the left-hand menu.
